To better ensure a successful visit, please bring your insurance card(s), photo identification, co-pay and all pertinent medical records as directed.

For your convenience, we have patient registration forms available on our website or via our patient portal for completion prior to your visit.  If you do not have an active patient portal account, please connect with any of our front desk staff members during your appointment to enroll.

Vaccines

Whether you are preparing for a surgical procedure, or just trying to stay healthy, we want you to have the best possible health. 

Arm yourself at least two weeks before your procedure with your annual flu or pneumonia vaccines. Speak with your pharmacy about Pneumonia vaccine recommendations.

Financial Policies

As a courtesy to our patients, we will file their insurance claims to help them receive the maximum reimbursement under their policy. Where there is an office co-payment or plan deductible due, patients will be expected to pay at the time of visit. Patients will be billed for any remaining balances after claims are processed and contract adjustments have been made.
